Planuojant pagal \u0161i\u0105 metodik\u0105, \u0161vitinamojo objekto pagrindinis t\u016bris yra gana didelis, nes \u012fvertinama maksimali\u0173 kv\u0117pavimo judesi\u0173 amplitud\u0117, \u0161vitinamo objekto jud\u0117jimo amplitud\u0117 j\u0173 ribose, paciento paguldymo ir kitos paklaidos. Kartu padid\u0117ja sveik\u0173 audini\u0173 ap\u0161vitinimo t\u016bris ir komplikacij\u0173 i\u0161sivystymo po spinduliuot\u0117s (pvz., spindulinio pneumonito (SP)) rizika [4].<\/p>\n\n\n\n<p>SP&nbsp;\u2013 nereta plau\u010di\u0173 spindulin\u0117 komplikacija. SP da\u017eniausiai i\u0161sivysto, kai yra taikomos didel\u0117s radioterapijos doz\u0117s arba ap\u0161vitinama didel\u0117 plau\u010di\u0173 sritis. Pirmiausia atliekamos planuojamos ap\u0161vitinti kr\u016btin\u0117s l\u0105stos srities KT, t.&nbsp;y. skersiniai rentgeniniai vaizdai, kuriuose radioterapeutas apibr\u0117\u017eia norim\u0105 ap\u0161vitinti ir saugom\u0105 audini\u0173 apimt\u012f (t\u016br\u012f)&nbsp;\u2013 plau\u010di\u0173 navik\u0105, kritinius organus, limfmazgius, tada skiria radioterapijos doz\u0119. Medicinos fizikai parenka atitinkam\u0105 \u0161vitinimo lauk\u0173 skai\u010di\u0173 (j\u0173 gali b\u016bti 2, 3, 4 ir daugiau), j\u0173 dyd\u012f, apskai\u010diuoja kiekvieno lauko spinduli\u0173 pluo\u0161to kritimo kamp\u0105 \u012f numatyt\u0105 ap\u0161vitinti srit\u012f, \u0161vitinimo laik\u0105 taip, kad visi numatyti ap\u0161vitinti audiniai tolygiai gaut\u0173 reikiam\u0105 spindulin\u0119 doz\u0119, o \u0161alia esantys sveiki organai b\u016bt\u0173 paveikiami kuo ma\u017eiau. Spindulinio gydymo planas ir visi jo parametrais i\u0161spausdinamas ant popieriaus ir saugomas kompiuterio programoje&nbsp;\u2013 juo kaskart, atlikdami \u0161vitinimo frakcij\u0105, vadovaujasi radioterapeutas ir jam padedantis technologas. Da\u017eniausia vienkartin\u0117 frakcijos doz\u0117 yra 1,8\u20132&nbsp;Gy, viso kurso&nbsp;\u2013 apie 60\u201370&nbsp;Gy&nbsp;[3, 10].<\/p>\n\n\n\n<p>V\u0117\u017eio gydymo efektyvumas priklauso ne tik nuo adekvataus naviko gydymo, bet ir nuo nepa\u017eeist\u0173 plau\u010di\u0173 audini\u0173 atsako \u012f j\u012f; nuo to priklauso komplikacij\u0173 da\u017enis ir sunkumas. Siekiant suma\u017einti komplikacij\u0173 po spinduliuot\u0117s rizik\u0105, \u012f praktik\u0105 \u012fdiegiamos naujos ST planavimo metodikos, kurias taikant galima maksimaliai efektyviai ap\u0161vitinti pagrindin\u012f navikin\u012f t\u016br\u012f ir suma\u017einti sveik\u0173 audini\u0173 ap\u0161vitinimo t\u016br\u012f. Viena toki\u0173 metodik\u0173\u00a0\u2013 4D ST planavimo technika. Tai yra 3D KT technologija ir kv\u0117pavimo judesi\u0173 sinchronizavimo galimyb\u0117. Tai galinga priemon\u0117 steb\u0117ti naviko ir sveik\u0173 organ\u0173 jud\u0117jimo b\u016bkl\u0119 planuojant radioterapij\u0105 ir jos atliekant\u00a0[11, 12]. 4D\u00a0KT yra gan\u0117tinai naujas Lietuvoje ST taikomas planavimo ir gydant naudojamas b\u016bdas. 4D KT technologija leid\u017eia \u012fvertinti naviko buvimo viet\u0105, t\u016br\u012f, santyk\u012f su gretimomis strukt\u016bromis ir kt. (analogi\u0161ka 3D KT technologijai), jo jud\u0117jimo amplitud\u0119 ir kit\u0173 k\u016bno organ\u0173 judesius. 4D\u00a0KT leid\u017eia tiksliai ir individualiai, pagal paciento anatomij\u0105 ir fiziologij\u0105, i\u0161matuoti naviko ir organ\u0173 judesius. 4D duomen\u0173 integravimas \u012f gydymo planavim\u0105 leid\u017eia suma\u017einti geometrinius neapibr\u0117\u017etumus, b\u016bdingus 3D\u00a0KT vaizdams. 4D\u00a0KT gauti duomenys panaudojami tiek rengiant \u0161vitinimo plan\u0105, tiek ir atliekant pa\u010di\u0105 proced\u016br\u0105.
